Summary
Non-Medical Case Managers (NMCM) have as their objective providing coordination, guidance and assistance in improving access to and retention in needed medical and support services to mitigate and eliminate barriers to HIV care services. The NMCM is responsible for all eligibility determination activities, enrollment/reenrollment in RW and assistance with determining eligibility for other benefits. The NMCM is responsible for performing an assessment of the client's need for Medical Case Management (MCM)as part of the intake/eligibility determination process.
Typical Duties
• Responsibility 1 (_70__%): Patient Assessment and Enrollment into grant funded services
o Task 1: Screen for initial eligibility for the Ryan White grant program, and annual renewals
o Task 2: Initial assessment and identification of various of service needs, referral to RW support
services, and determination of referral to MCM
o Task 3: Development of a comprehensive, individualized service plan.
o Task 4: Coordinate access to medically appropriate levels of health and support service and
continuity of care
o Task 5: Advocacy and/or review of utilization of services
o Task 6: Continuous client monitoring to assess the efficacy of the care plan, and reevaluation of
the care plan at least every 6 months.
o Task 7: Continual assessment of the client's and other key family members' needs and personal
support system.
o Task 8: Ensure any and all documents related to the eligibility screening process are scanned into
EPIC, the electronic medical record.
• Responsibility 2 (_15__%): Data Entry
o Task 1: Collect data and enter into CAREWare data system in accordance with the time frame as
stated in the CAREWare Data System Policies and Procedures as set forth by the Data Manager
o Task 2: Assist in data clean-up before annual/monthly report submissions (RSR)
o Task 3: Work collaboratively with the Data Manager to ensure appropriate data collection
methods are in place for Non-Medical Case Management and the service referrals provided
• Responsibility 3 (_5__%): Quality Management
o Task 1: Participate as required in Quality Management and Quality Improvement activities
o Task 2: When requested, attend meetings as set forth by the Quality Manager
o Task 3: Work collaboratively with the Quality Manager to ensure appropriate quality measures
are in place for Non-Medical Case Management, for example: Patient Notification and
documentation regarding incomplete eligibility and/or denial to grant funded services
• Responsibility 4 (_5__%): Other Duties as Assigned
o Task 1: Attend all monthly team meetings prepared to discuss and achievements or opportunities
for improvement related to your assigned duties
o Task 2: Attend all annual team retreats
o Task 3: Attend any supervisor-required meetings, trainings, competencies
o Task 4: While Working away from the office for an approved reason, conduct one-self as an
ambassador of the University and adhere to all ethical and conduct roles as set forth by the University
• Responsibility 5 (_5__%): Professional Development/Continuing Education
o Task 1: Attend or complete at least one pre-approved professional development/continuing
education activity through the VCU Ryan White Programs Continuing Education (CE) Professional
Development Policy
o Task 2: Provide feedback to peers and program leadership upon completion/attendance of preapproved professional development/continuing education activity
Minimum Qualifications
• Demonstrated experience working in and fostering a diverse faculty, staff, and student environment or commitment to do so as a staff member at VCU • High School diploma • Knowledge of Medicare, Medicaid, and other state or federal funded assistant programs for eligible
individuals
• Knowledge of HIPAA and ability to implement privacy and confidentiality practices
• Excellent customer service and interpersonal skills
• Excellent time management and organizational skills
• Ability to demonstrate superior teamwork behaviors
• Ability to work independently
• Proficient in MS Office
• Sensitive to confidential health information and HIV/AIDS related issues and controversies
• Outstanding communication skills, oral and written
• Cultural competency in working with a diverse population
Preferred Qualifications
• Bachelors or Associates Degree preferred
• Experience in working in a healthcare delivery system in a customer focused position
• Knowledge and Experience with the Ryan White grant program
