Position Description

Certified Nursing Assistant

REPORTS TO:   Charge Nurse

POSITION:         Provide personal care and supervision to residents in a manner conductive to their safety, comfort, security and the greatest degree of independence possible.

Pay: $19.00 an hour, plus .20cents for every year of experience up to 20years.  Shift differential is additional.

POSITION QUALIFICATIONS:

  • Current, valid certification as a Nursing Assistant in the State
  • Must be able to pass a DHS/DIA background check

 

PHYSICAL REQUIREMENTS:

Standing:                     100% of day

Walking                       3-8 hours per day

Lifting:                        Over 50 lbs. good body mechanics or 10-50 lbs. improper body

Mechanics

            Twisting:                     Moderate: 121-480 twists per day required

            Bending:                     Frequent bending: 481 or greater bends per day

            Squat/Kneel:               3 – 7 hours per day

            Endurance:                  Heavy energy requirements (7 – 9 mets)

            Wrist Position: Slight deviation of wrist

            Pinching:                     Moderate – 121 – 480 pinches per day

            Hand/Wrist Reps:       Moderate – 961 – 2880 per day

            Manual Dexterity:       Gross dexerity6 with infrequent find motor movement

            Right Hand:                36 -60% of job cycle time

            Left Hand:                  36 -60% of job cycle time

            Near & Far Vision:      Requires minimal near and minimal far vision

            Hearing:                      Requires hearing whispered voice at 8 feet (FAA Class II)

            Depth Perception:       Accurate depth perception required

WORK ENVIRONMENT:

  • Works primarily indoors in a well-illuminated, reasonably climate-controlled environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Resident Care

  • Assumes accountability for compliance with Federal, State and other regulations.
  • Assists in the implementation of each resident’s care plan by performing personal cares and all Activities of Daily Living (i.e., dressing, feeding, toileting, grooming).
  • Answers any call light to determine resident needs and take appropriate action.
  • Notify charge nurse of change in resident’s condition including skin tears, bruises, reddened areas, etc.
  • Assist with bed bath, tub bath or whirlpool.
  • Perform personal cares (i.e., nails, hair, and oral hygiene).
  • Assist residents in maintaining a clean, well-groomed appearance.
  • Assist with feeding resident; serve meal trays.
  • Assist with AM and PM cares.
  • Assist with weighing of residents
  • Measure and record resident intake and output.
  • Assist residents with applying braces under direction of licensed personnel with appropriate training.
  • Pass fresh drinking water and nourishments each shift and PRN.
  • Assist in placing and removing protective devices and document on restraint sheet daily as directed.
  • Complete and submit daily assignment sheets and give Charge Nurse daily report.
  • Observes and verbally reports to the charge nurse accurate and timely information concerning residents. Reports changes in medical condition of health status, lack of response or resistance to care, special problems, unusual incidents, residents needs or requests, behavioral or psychological concerns. Documents in writing according to established procedures or as requested.
  • Complies with the resident’s care plan in the performance of Rehabilitation, Range of Motion, or periods of exercise as ordered by the physician, PT, etc.
  • Complies with safe lifting, transferring and handling of resident per procedures. Assumes responsibility for the safety of residents under immediate supervision and those assigned at any time.
  • Participates in shirt report change.
  • Assists with admission, discharge, and transfer of residents. Documents according to established procedures.
  • Listens to residents and families/staff questions and concerns and responds in a timely manner. Reports any irresolvable concerns to supervisor.
  • Assists with transporting residents to activities and beautician appointments.
  • Assures that resident’s rooms are maintained in a clean orderly fashion (i.e. make beds, etc.).
  • Must care for resident needs

Safety and Sanitation

  • Observes safety hazards and emergency situations, and reports to appropriate person or takes corrective action according to established procedures, works safely without danger to self or others.
  • Assumes accountability for using, maintaining and storing facility property according to established policy.
  • Demonstrates knowledge of emergency policies and procedures.
  • Performs all duties in a safe and efficient manner; assures that safety regulations are followed at all times by all staff.

Customer Service

  • Interacts with residents, families, staff, and other visitors to the facility in a pleasant, respectful, and courteous manner.
  • Acts as a positive representative of the facility at all times.
  • Performs incidental housekeeping and maintenance tasks as may arise during the course of regular duties, in order to maintain a clean, safe, pleasant environment for residents, visitors and staff.
  • Responds to any and all call lights within 5 minutes as applicable.

Residents/Patients’ Rights

  • Provides privacy and maintains the confidentiality of all resident care information.
  • Ensures care and security of residents’ personal possessions.
  • Reports all complaints and grievance made by residents, family members, and visitors to the supervisor.
  • Ensures that residents are free from abuse (physical, mental, and sexual), mistreatment and neglect, and reports any such instances to appropriate state agencies.

In-Service Education

  • Attends all in-service education programs mandated by federal, state, and company guidelines.
  • Participates and assists in departmental meetings and projects as assigned.

 

Attendance

  • Observes guidelines as established by facility personnel handbook.

Miscellaneous

  • All other duties assigned by the supervisor.
